The Mental Health Integrated Complex Care (MHICC) program can provide you with flexible, personalised support including:
- clinical nursing services
- care coordination and liasion with clinical and non-clinical care providers
- family support and liasion
- improve your access to psychiatrist and psychological care
MHICC is funded by South Eastern Primary Health Network (SEMPHN) and delivered by Mind Australia.
Who for:
People with severe mental illness who are in the greater Dandenong area who have complex needs and will benefit from improved coordination of their care over an extended period (6-12 months). This program is also for people who cannot access similar supports under the NDIS.
How to access:
Access is via the South Eastern Melbourne Primary Health Network (SEMPHN) access and referral team - call 1800 862 363 during business hours. Refer to MHICC services here.
