Mental Health Integrated Complex Care (MHICC)

The Mental Health Integrated Complex Care (MHICC) program can provide you with flexible, personalised support including:

  • clinical nursing services
  • care coordination and liasion with clinical and non-clinical care providers
  • family support and liasion
  • improve your access to psychiatrist and psychological care

MHICC is funded by South Eastern Primary Health Network (SEMPHN) and delivered by Mind Australia.

Who for:

People with severe mental illness who are in the greater Dandenong area who have complex needs and will benefit from improved coordination of their care over an extended period (6-12 months). This program is also for people who cannot access similar supports under the NDIS.

How to access:

Access is via the South Eastern Melbourne Primary Health Network (SEMPHN) access and referral team - call 1800 862 363 during business hours, or fax a referral form to 1300 354 053.
Referral forms are available from the SEMPHN website (www.semphn.org.au/mhintake).
