Sub-acute residential recovery care
Prevention and Recovery Care (PARC) - FRANKSTON
With sub-acute, short stay step-up and step-down care (delivered through partnership with Peninsula Health) residents can access specialist clinical mental health staff, while also being supported to strengthen relationships with family and friends, and skills for living safely in the community. PARC provides a schedule of group activities alongside individual 1:1 support based on your strengths and goals as identified in your individual recovery plan. Frankston PARC is an adult service (25 - 64yrs).
Who for:
People who need short term intensive recovery support to help them transition out of an inpatient ward or to help them avoid hospitalisation.How to access:
Access is generally through Peninsula Health and facilitated through psychiatric assessment. This may be via Crisis Assessment and Treatment Team (CATT), Continuing Care Team (CCT) or similar. Eligibility is contingent on Area Mental Health Service case management.Outcome statement:
